Assessment:
We will explore your passions and interests while examining potential road blocks or barriers to participation. Short- and long- term goals will be created based on areas of need within the 5 health domains: physical, cognitive, social, emotional, and spiritual.
Planning:
Together we will create a plan to bring positive change into your life. This phase includes the identification of goal-based interventions and resources.
Implementation:
I will support you through each step to achieve your goals. You will begin to experience the health benefits and positive changes of a healthy leisure lifestyle.
Evaluation:
Throughout the process we will determine what works for you, what doesn't, and why... then adjust your plan accordingly.
